Nomination Rules
First, download our submission template (blank, formatted MS Word document) and complete the form. You will need to complete a separate document for each award category in which you want to nominate your company. Provide information about why your company deserves to win the award in the category. (See the suggested topics section for items you may wish to include in your submission.)
Please complete just one nomination form for each category. The Word template is setup with the font size and type (Arial 12 pt) and the margins (1/2" all the way around) for the document. This format must be used. The final document should not be longer than 4 pages in this exact format. Photos and/or graphics may be included within the four pages, but no additional documents or attachments will be accepted beyond these four pages.
Be sure to save the completed template form with a unique identifying name (Company Name - Diversity, Company Name - Work Environment and Scheduling, or Company Name - Health and Benefit Programs). This will be attached to your completed form below. IMPORTANT: Complete the nomination form below and submit it electronically. You will only need to complete ONE nomination form!

Step-by-step...
- Download the submission template (a formatted MS word template)
- Complete a submission document for each category your company is entering. Explain why your company should receive an award.
- Save each submission documents separately using a unique name (eg. Company Name – Diversity, Company Name – Work Environment and Scheduling, Company Name – Health and Benefit Programs)
- Complete the nomination form below and click “submit” when you are finished. (You do not need to submit more than one nomination form.) You will be taken to a screen to upload your nomination submission document.
